Enfield Council’s Adult Social Care Service is proud to introduce 5 new Prevention and Early Intervention Services. These services are designed to support our community by promoting independence, offering vital information and advice, and providing early help to keep people healthy and well. From assisting unpaid carers to helping individuals return home after hospital stays, these initiatives aim to make a real difference in people’s lives.
Enfield Prevention and Early Intervention Services are designed to support you to maintain independence and improve well-being and these services are delivered in partnership with Enfield’s voluntary and community sector. The overarching goal is to provide timely, person-centred information and support to empower residents to live healthier, more connected, and independent lives.
Home from hospital
Alpha Care Specialists
Facilitate recovery from illness and successful hospital discharge through the provision of high quality support in the home, so that people are confident and able to discharge from a hospital setting and live independently in the community.

Services provided under this contract
This is a whole-system partnership model of integrated care. It ranges from supporting people who are discharged from the acute local hospitals. Support to people is provided to ensure they settle safely within their homes and have access information and advice and community support.
This free service is for people living in the Borough of Enfield who are leaving hospital and require some extra help and support.
We can help with light household tasks, meal preparation, medication collection. We can also signpost service users to other organisations to provide tailor made support depending on their needs.

Unified Community Assistance Network (UCAN) is a partnership of local voluntary sector organisations working together to meet the specific needs of local residents. Whether you're dealing with isolation, mental health issues, or need help managing your health, we have something to offer from across the network of trusted partners:
- Mind provides mental health support and early intervention
- One to One supports autistic individuals and those with learning disabilities, reducing isolation
- Middlesex Association for the Blind enhances accessibility for individuals with visual impairments
- Wellbeing Connect offers integrated support for families with complex needs, focusing on underrepresented communities
- Cooking Champions leads healthy eating workshops to reduce diet-related conditions
- Bread n Butter runs cookery workshops to promote healthier lifestyle choices
- Age UK Enfield, as the lead organisation, also directly delivers services such as falls prevention, mobility support, and active ageing programmes
We work with residents of Enfield who are aged 18+ with long term health conditions, sensory loss/impairment, neurodiversity, older people and those experiencing mental health concerns or loneliness. We will work with you one to one to help you work on plan to take steps to improve your health and wellbeing. You can access a wide range of activities ranging from sports and physical activities, to cookery courses or eat and share lunches. Our aim is to help you to feel healthy, confident, move more and connected to the community, to live and age well.
One to one support planning and goal setting, information and advice on local services and support, healthy eating and nutrition programme, inclusive exercise programme, peer support wellbeing sessions and Adult Autism Hub.

Advocacy and inclusion
Local Enfield Advocacy Partnership (LEAP)
The Local Enfield Advocacy Partnership (LEAP) provides independent advocacy, enabling you to express your views, understand your rights, and participate in decisions. The partnership provides one-to-one advocates as well as peer and group advocacy to make sure you are supported to be independent and have choice and control. The service is inclusive and works closely with Enfield’s Adult Social Care Partnership Boards to ensure local residents’ voices are heard in shaping local policy and services. The service empowers people to self-advocate and promotes equality, dignity, and social justice.

The Local Enfield Advocacy Partnership (LEAP) can support you in many areas.
Understanding your rights as service user or carer
Support with appealing welfare benefit decisions (in respect of benefits you are already in receipt of – for support to apply for welfare benefits, please contact DANE).
Providing representation to help have your views, wishes and voice heard through individual (one to one) advocacy, peer advocacy and group advocacy.
Individual and community representation
Training, access to information on rights, peer support, developing community advocacy expertise.
Understanding the assessment and care planning process
Support to navigate and access services including health, social care, community services.
Developing knowledge and understanding, to help build confidence when engaging with professionals and other services.
Group training and advocacy
Support to access consultation opportunities to help influence design and development of local services.
We will also be hosting the ‘Enfield Empowerment Network’, this is a network of community organisations working together to promote advocacy, identify issues and strategies to overcome them in the borough.

Information, advice, digital inclusion and assistive technology
DANE (Disability Advice Network Enfield)
Ensure people have access to the clear and personalised information and about key health, social care, and Voluntary Community Sector services. This will enable people to make informed decisions about the services they receive to support their independence, health, and wellbeing.

What DANE Will Deliver
Through DANE, residents with disabilities will have access to:
- Free, impartial information and advice on disability rights, services, and independent living
- Digital inclusion support, including 1:1 training and group sessions to build confidence with technology
- Assistive technology guidance, helping residents safely and confidently use devices that support daily life
- Community-Based Outreach – Local drop-ins at the 4 community hubs, Digital and wellbeing navigation services, and peer support to make help easy to find and access
The aim is to reach residents earlier, help prevent crisis, and support long-term independence, inclusion, and resilience.
