Before booking, is this service still needed?
Do any of the following apply to you?
- I have recently had a mobility or home equipment assessment from another service and no longer need support from Adult Social Care
- I have arranged mobility equipment privately since my referral
- My mobility has improved since my referral and I am no longer concerned about my mobility or risk of falling
- I am seeking an occupational therapy assessment to support a rehousing application
This service is not suitable for your needs.

This service is designed to assess needs for minor adaptations and equipment. If your circumstances have changed since your referral, we will need to review your support needs by phone before arranging an appointment.
